The nurses at HCR Home Care work with you and your physician to plan and oversee your home care. From a comprehensive initial assessment, our nurses develop a treatment plan for your medical conditions and help you become independent with your disease management through education and support.
This may include:
- Assistance with medication management
- Wound care
- IV therapy
- High Blood Pressure
- Care assessment and coordination
HCR’s Director of Clinical Excellence is responsible for ensuring HCR’s practitioners are competent in new science as it emerges in the practice. HCR works with nurse leaders and nurse scientists across the country as translational research partners. These relationships have had a positive impact on our ability to have an evidence-based practice.
