Care Management is an approach to care focusing on Patient Education and coordination of services that promote self-management.
Our Care Managers routinely work with work with Department of Human Services, medical providers, insurance companies, community resources, and housing programs to help meet our clients needs.
The role of a Care Manager is to work in partnership to identify goals, acknowledge needs, and to refer and connect to necessary services.
Care Management focuses on physical, behavioral, and social health goals.
Self sufficiency and advocacy are the goals of Care Management. Care Management is a success when our partnership identifies client needs, puts appropriate care services in place, and creates positive lasting change.
Care Managers will:
► Identify barriers to reaching goals assist in creating a crisis plan.
► Identify gaps in care and assists to get medical needs met
► Advocate to make sure your care team is working for you for
hospitalization discharge, home care services, or medical specialists.
► Find community resources such as food pantries, clothing closets,
and community programs
► Schedules medical transportation
► Provides housing lists and helps fill out applications
