HCR provides pediatric home care services for medically fragile and technology supported children and has nursing staff experienced in pediatric care in our Madison, Cortland, Schoharie, Monroe, Genesee, and Orleans offices.
Our approach to pediatric home care centers on coordination of care prior to hospital discharge ensures a safe transition to home. A dedicated, hospital-based, Care Transitions Manager works with the hospital team and parents/caregivers to prepare the home environment for the unique care needs of an infant or child.
A multidisciplinary team of experienced pediatric clinicians work with your Pediatrician and other specialists to ensure care at home is of the highest standard. Consulting Pediatric Nurse Practitioners will be retained as needed to provide our staff with the necessary clinical resources and support.
HCR nurses and therapists have experience in caring for infants or children at home with health challenges, including the following:
► Tracheotomies ► Bilirubin therapy
► Ostomies ► Nasogastric feedings
► Infusion ► Gastrostomy tubes
► Apnea monitoring ► Seizure disorders
► Synagis injections ► Developmental disabilities
Additionally, many of our nurses have inpatient, acute care experience in NICU, Pediatrics and OB and have transferred their expert assessment skills and familiarity with technology to the community setting.
HCR also has Physical, Occupational, and Speech Therapists who have experience and/or certification in the care and treatment of infants and children. The Team’s Occupational Therapist is certified in Pediatric Neurodevelopmental Treatment and Early Intervention, and serves in a leadership and consulting role to other clinicians.
Medical Social Workers are integral to the home care team by providing support, counseling, and referrals to community resources and teaching coping mechanisms to parents or caregivers struggling with a medically fragile child.
In addition to providing care for medically fragile children, our nurses are experienced providers in the MOMS program, and give education to new, young or at risk mothers, focusing on well baby care, nutrition, growth and development.
